Please read carefully and be aware that in registering yourself or your minor child/ward for participation in volunteer service, you will be waiving and releasing any and all claims for injuries you or your child/ward might sustain arising out of the volunteer service provided. As a guest of the New Lenox Community Park District, you agree to abide by all rules/policies of the New Lenox Park District. You understand that your failure to abide by said rules/policies may result in you being asked to leave the Facility/Park. You recognize and acknowledge that there are certain risks of physical injury while volunteering, and you agree to assume the full risk of any such injuries, including death, damages or losses regardless of severity, which you or your child/ward may sustain as a result of volunteering in said activities. You waive or relinquish all claims you or your child/ward may have against the New Lenox Community Park District and its Officers, Commissioners, Agents, Servants, and Employees, and any other cooperative Park District and its Officers, Commissioners, Agents, Servant, and Employees, as a result of volunteering in such activities. You further agree to indemnify and hold harmless and defend the Park District(s) and its Officers, Commissioners, Agents, Servants, and Employees from any and all claims resulting from injuries, damages and loss sustained by you or your child/ward, arising out of, connected with, or in any way associated with the volunteering activities in which you or your child/ward are involved in.
